Please type your CONTACT NAME below.   This will serve as your electronic signature agreeing to the following terms: Applicant, in the use of booth space, shall hold harmless Rhoads Garden Center, Inc. and any and all owners and employees of aforementioned companies/organizations, against any and all claims for personal injury, illness, death, property loss and/or damage occurring directly or indirectly from use of the booth space.  By signing this contract, you also acknowledge and agree to the terms of this contract and that you will obtain and maintain any licenses, permits, and other documentation required for the operation of your booth.  By signing below, I also acknowledge that if I have not supplied visual images of BOTH my products and my displays, that my application will not be considered, that Winterfair is held rain or shine, that my space will be forfeited to another vendor if my deposit is not paid upon acceptance and my remaining full fees are not paid by the due date listed on my invoice, that no refunds will  be given to applicants who cannot attend the event.*
